Your First ConsultationTo make an appointment with The Park Clinic Orthopaedic Surgeons, you must have a referral letter from your doctor.
Appointments can be made by telephoning each surgeons' rooms on the numbers shown on below.
What to bring:To meet with an orthopaedic surgeon from the Park Clinic, you must first receive a referral letter from a registered doctor. Please remember to bring this letter, along with all other information supplied by your referring doctor, including reports, x-rays and MRI scans. - If you forget any of this information, you may not be able to consult with your specialist.
It may also be useful to bring any information relevant to your medical history, such as current medication, allergies and any ongoing medical problems.
When to arrive:It is best to arrive at least 15 minutes prior to your appointment, so that you have time to fill out paperwork such as your New Patient Registration Form. The Park Clinic advises patients to download and complete their New Patient Registration form prior to arriving for your first appointment.
